I, [Customer's Full Name], hereby acknowledge and agree to the following terms and conditions before participating in any yoga classes or activities at Find Your Center Yoga Studio (hereinafter referred to as the "Studio"):

1. Health and Medical Information: I affirm that I am in good physical condition and have no medical conditions, injuries, or physical limitations that would prevent me from safely participating in yoga classes at the Studio. I understand that it is my responsibility to consult with a healthcare provider before beginning any exercise program.
    

2. Assumption of Risk: I acknowledge that participating in yoga classes involves certain inherent risks and hazards, including but not limited to physical exertion, stretching, and the use of yoga equipment. I understand and accept these risks and voluntarily choose to participate in classes at my own risk.
    

3. Release of Liability: In consideration for being permitted to participate in classes at the Studio, I hereby release, discharge, and hold harmless the Studio, its owners, instructors, employees, and agents from any and all claims, liabilities, injuries, or damages, including but not limited to those arising from negligence or misconduct, that may arise from my participation in yoga classes or activities at the Studio.
    

4. Photography and Videography: I grant permission to the Studio to photograph or record me during classes for promotional or instructional purposes. I understand that my image or likeness may be used in Studio marketing materials, website, or social media. If I do not wish to be photographed or recorded, I will notify the Studio in writing.
    

5. COVID-19 Considerations: I acknowledge that the Studio may have COVID-19 safety protocols in place, including but not limited to mask requirements, social distancing, and enhanced sanitation measures. I agree to comply with these protocols and understand that the Studio is not responsible for any exposure to or contraction of COVID-19 during my visit.
    

6. Payment and Cancellation: I understand that I am responsible for the payment of all fees associated with my participation in classes at the Studio. I also understand and agree to the Studio's cancellation and refund policies as outlined in their terms and conditions.
